Pregnancy Kit FAQ's

You may apply for the MomWell Care Kit as soon as your Provider has diagnosed you as pregnant, and you have become established in your Provider office as a patient. This is required as your Provider will need to complete the order form for you to obtain the kit items.

Only the individual who is diagnosed as pregnant can fill out the online order submission. All the filled in information must be for the pregnant individual, and the page 2 consent must be signed by the pregnant individual.

The MomWell Program was developed by Synergy DMEPOS to address the
symptoms experienced during pregnancy and the postpartum period and to
assist in the reduction of serious conditions such as VTE.


Working with OBGYN providers, Synergy has established a Program that
allows providers to offer therapeutic support to pregnant patients through
an organized, administratively simple process. The Program identifies and
addresses common symptoms, during the perinatal period, experienced
due to the normal physiologic changes that occur during pregnancy and
postpartum that create pain and discomfort, and in some cases result in
serious complications and ultimately hospitalizations.

MomWell offers a solution that was not previously available, in a Program
format, to support maternal wellness in a systematic fashion providing the
appropriate support items at the relevant time during the perinatal
experience.

The standard MomWell Care Kit includes six (6) FDA-cleared medical
devices, distributed via three separate shipments, to be utilized at the
appropriate time during the perinatal period.

Yes. The standard MomWell Care Kit does include a breast pump that is
delivered in the third shipment, at approximately 27 weeks of gestation.


Once your MomWell Rx Form is received from your treating provider, your
insurance eligibility and benefits are verified, and you will be contacted, via
phone or email, by our breast pump partner, MedSource, to provide you
with a choice of breast pumps available to you through your insurance. You
will be provided with a list of breast pumps that are fully covered as well as
additional options that may be purchased through an upgrade option.


Your chosen breast pump will be shipped to your home, along with the 3-
in-1 postpartum compression garment to be used following delivery.

Yes. The breast pump may be excluded from the standard MomWell Care
Kit by striking the device from the Rx Form and/or by responding “no
interest” to the question related to breast pump interest on the online
request form.

The sizing chart is included below and illustrates the technique for proper
measurement and sizing.

As an alternate means of determining product size/fit, you may select your
pre-pregnancy clothing size as this sizing methodology has proven to be
consistently accurate.

Should you determine that the selected size does not fit properly, you may
contact our customer service department to exchange the item for the
correct size. Synergy DMEPOS will provide you with the correctly sized item
and a return label to complete the exchange.

Yes. The MomWell Care Kit is comprised of 6 medical devices that require a
valid provider order to supply to a patient. As part of the Synergy DMEPOS
order management process, Synergy DMEPOS will work with your OBGYN
provider to obtain a valid prescription and required documentation.

The MomWell Care Kit may be obtained through an order generated by
your provider during a routine pregnancy office visit, or if you have
completed an online request for the MomWell Care Kit, our MomWell
customer service team will reach out to your OBGYN provider to obtain the
required, valid prescription.

In both scenarios, the Synergy team will and must obtain the required
prescription and documentation to fulfill the order and bill your insurance
plan.

Yes. The MomWell Care Kit is covered by most health insurance plans. As
part of the order management process, Synergy DMEPOS obtains your
health insurance information and written authorization to contact your
health plan. Your eligibility and benefits are verified prior to fulfillment of
your order.

If you have submitted an online website submission request for our kit,
please make sure to sign the DocUSign that is emailed to you following your
website submission. This signed DocUSign document grants Synergy
DMEPOS the permission to verify your insurance eligibility and benefits.

Yes. Synergy DMEPOS will verify your eligibility and benefits with respect to
coverage of the MomWell Care Kit. Prior to shipping the Kit, we will notify
you of any issues related to coverage as well as any financial responsibility
on your part.

You will not receive an unexpected bill from Synergy DMEPOS after the Kit
has been shipped.

Additionally, following receipt of your MomWell Kit, should you receive an
Explanation of Benefits (EOB) from your insurance carrier which indicates
they have received the claim/bill for the products. This document in some
cases will initially reflect a denial for an item. THIS IS NOT A BILL. Synergy
also received this notice which drives our appeal process to provide
additional information to obtain coverage. Many plans cover the item when
additional information is provided by our billing team. Again, this is a
notice, and you are NOT being billed for this product. If you receive this and
have any questions, please contact Synergy directly.

Yes. Your electronic signature is required to provide Synergy DMEPOS with
the authorization to contact your insurance company, to verify your
benefits, and reach out to your OBGYN provider to obtain a valid
prescription to fulfill your MomWell Care Kit order.


Your electronic signature does not authorize us to ship the Kit without
notifying you of your coverage benefits.

The MomWell Care Kit will be shipped to your home in three (3) separate shipments based on the appropriate time of usage for the products in each delivery.
 
The first shipment will arrive at your home approximately two (2) weeks following Synergy’s receipt of the complete and valid MomWell Care Kit prescription form and required documentation.
 
The second shipment will arrive approximately 2 to 3 weeks following the first shipment.
 
The third and final shipment will be shipped to you at approximately 27 weeks of gestation, or at the time frame specified for coverage by your health insurance plan.
 
The above time frames are based on provision of the kit at approximately 18 to 22 weeks gestation. If you, or your provider, initiates the order later in your pregnancy the time frame for delivery will be condensed to ensure you receive the items during the appropriate time for usage.
